Responsibilities
- Administer medications and treatments per physician orders; document in the chart.
- Monitor resident condition; perform assessments; communicate changes to the supervising RN.
- Provide wound care, hygiene support, and routine nursing procedures.
- Assist residents with activities of daily living; support safety and comfort.
- Collaborate with the interdisciplinary team to coordinate care and contribute to care planning.
- Complete accurate charting and handoffs; maintain MAR accuracy in electronic health records; respond to call lights as needed.
Requirements
- Active LPN license in Vermont (in good standing)
- Availability to work evening and night shifts
- Experience in long-term care or geriatric nursing preferred
- Strong communication and documentation skills; team-oriented
- Physical ability to perform duties in a long-term care setting
