Responsibilities
- Conduct thorough OT evaluations for adult residents in a skilled nursing facility, review referrals, identify mobility, ADL, cognitive, and upper-extremity function needs, and establish measurable goals.
- Plan and implement individualized treatment interventions aimed at improving activities of daily living, mobility, dexterity, cognition, sensory processing, and adaptive equipment recommendations.
- Reassess resident progress regularly, document treatment plans and progress notes in the medical record, and adjust goals in collaboration with the care team.
- Educate residents and families on safety strategies, energy conservation, activity modification, and use of adaptive equipment to support independence in daily routines.
- Collaborate with a multidisciplinary team including physical therapy, speech-language pathology, nursing, and social work to coordinate care and support effective discharge planning.
- Ensure compliance with regulatory standards and facility policies, maintain accurate documentation, and participate in quality improvement initiatives when applicable.
Requirements
- Active Occupational Therapist license in New Hampshire or eligibility for licensure by endorsement.
- Experience or demonstrated comfort providing OT services to adults in long-term care or skilled nursing settings.
- Strong assessment, treatment planning, and documentation skills; proficiency with electronic health records or equivalent systems.
- Excellent communication and collaboration skills; ability to work effectively within a multidisciplinary team and with residents and families.
- Ability to meet the assignment schedule and work in a skilled nursing facility; willingness to complete required background checks and comply with facility policies.
