Responsibilities
- Evaluate residents to determine physical therapy needs and goals
- Develop, implement, and adjust individualized treatment plans
- Administer evidence-based PT interventions for gait, strength, balance, and mobility
- Reassess progress regularly and modify plans accordingly
- Document assessment findings, treatment provided, and progress in medical records
- Collaborate with an interdisciplinary team to optimize patient outcomes
Requirements
- Active Physical Therapy license to practice in Maryland (or eligible for licensure in Maryland)
- Doctor of Physical Therapy (DPT) degree from an accredited program
- Ability to work in a skilled nursing facility setting
- Experience or interest in geriatric rehabilitation preferred
- Strong documentation and communication skills; team-oriented approach
- Eligible to participate in a travel assignment with the listed schedule
