Licensed Practical Nurse, Long-Term Care, South Burlington, Vermont, United States. Travel assignment starting 05/26/2026 for several weeks with 40 guaranteed hours per week. Responsibilities
- Provide direct nursing care to residents in a long-term care setting according to individualized care plans.
- Monitor residents’ condition, document vital signs, assess changes, and report abnormalities to the supervising RN.
- Administer prescribed medications and treatments; observe and respond to adverse effects or medication interactions.
- Perform routine LTC procedures (wound care, injections, catheter care, specimen collection) as delegated.
- Assist with activities of daily living, mobility, hygiene, nutrition, and safety to support resident comfort and independence.
- Communicate clearly with residents, families, and the care team; document care and changes accurately in the charting system.
- Ensure adherence to infection control, safety protocols, and privacy standards; participate in care planning and quality improvement activities.
Requirements
- Active Licensed Practical Nurse (LPN) license in Vermont (or eligible to practice in Vermont per licensure rules).
- Current CPR/BLS certification.
- Completion of an accredited practical nursing program (practical nursing diploma or equivalent).
- Prior experience in a long-term care or skilled nursing setting preferred.
- Ability to work evening and/or overnight shifts and adapt to a fast-paced environment.
- Ability to pass standard employment screenings (background check and drug screen) and meet facility-specific requirements.
