Travel Occupational Therapist Job
Occupational Therapist travel position in home health care, San Ramon, California. Start date 05/04/2026 for 26 weeks. Weekly pay range: $2,288 per week to $2,348 per week. Guaranteed hours: 36 per week. Shift: Days. This is a travel - temporary assignment in a home health setting.

Responsibilities
- Conduct initial and ongoing occupational therapy evaluations in the home setting and develop individualized plans of care
- Implement evidence-based OT interventions to improve activities of daily living, upper-extremity function, mobility, and cognitive processing
- Assess home safety, recommend adaptive equipment, and guide environmental modifications to support independence
- Document progress and update the plan of care; complete required OT documentation including OASIS-related assessments and discharge planning
- Communicate with clients, families, and the interdisciplinary team to coordinate care and scheduling
- Travel to client homes within the assigned territory; meet visiting targets and be prepared for potential weekend visits as required
Requirements
- Active California OT license
- ACOTE-accredited OT degree
- BLS certification
- Valid driver's license with auto insurance and vehicle registration
- TB (annual test), MMR, Varicella, HepB, TDap (within 10 years), and annual flu vaccination
- Clear drug screen and physical; willingness to see Covid+ and Covid PUI patients as required
- New graduates welcome; home health experience preferred but not required
- Ability to work independently in a home-based setting and manage a flexible schedule
