Responsibilities
- Conduct initial physical therapy evaluation in a home health setting and develop an individualized plan of care.
- Administer PT interventions focused on functional mobility, gait training, balance, strength, flexibility, and pain management according to the plan of care.
- Reassess patient progress, modify the plan of care as needed, and document outcomes and changes.
- Ensure accurate, timely documentation in OASIS and Epic EMR; manage data entry and compliance with program requirements.
- Educate patients and caregivers on home exercise programs, safety, and energy conservation; coordinate with the interdisciplinary team for comprehensive care.
- Maintain patient safety, infection control, and professional standards during home visits; communicate any changes in patient status to the care team.
Requirements
- 1+ year of home health physical therapy experience
- Proficiency with OASIS documentation and Epic EMR
- Active physical therapy license in good standing or eligibility (state-specific)
- Ability to work in a home health setting and travel to patient locations
- Availability to start on 07/22/2026 for a 12-week assignment
