Responsibilities
- Administer prescribed medications and treatments per physician orders and facility policy.
- Monitor residents’ conditions, document changes, and report to the supervising RN or physician.
- Assist with activities of daily living and mobility to support resident comfort and safety.
- Maintain accurate, timely documentation of care provided in the charting system.
- Collaborate with the interdisciplinary care team to develop and update individualized care plans.
- Adhere to infection control practices and safe patient handling protocols.
Requirements
- Active Licensed Practical Nurse (LPN) license.
- Basic Life Support (BLS) or CPR certification.
- 1 year of long-term care or skilled nursing facility experience preferred.
- Completion of an approved practical nursing program (diploma or certificate).
- Ability to work evenings or nights as required by the assignment; commitment to a long-term care setting.
