Setting: Home Health with service coverage across Grass Valley/Yuba County and surrounding counties; primary need currently in Butte County. Mileage is paid in addition to the bill rate. This HOT NEED requires a licensed PT with either current home health experience or willingness to train for a short period. DPT is required if obtained after 2016. BLS certification is required. Responsibilities
- Assess, evaluate, and treat patients in a home-based setting using standard physical therapy procedures
- Develop, implement, and monitor individualized plans of care (POC) to improve mobility, strength, balance, and function
- Provide therapeutic exercise, gait and transfer training, patient and caregiver education, and safety counseling
- Reassess patients regularly, document progress, and adjust POC in collaboration with the care team
- Coordinate care with physicians, nurses, and other providers; document thoroughly in the chart and ensure HIPAA compliance
- Travel to patient homes across designated counties; manage equipment needs and safety considerations for home environments
Requirements
- Active Physical Therapist license in California
- Doctor of Physical Therapy (DPT) required if obtained after 2016
- BLS certification
- 21 years of age or older
- Experience: 1 year home health experience preferred; new grads considered with training
- Ability to travel within the service area and work in home health environments
- Strong documentation skills and ability to work independently within a travel schedule
