Travel Physical Therapist Job

Responsibilities
- Assess patients to determine PT needs and establish individualized plans of care.
- Deliver therapeutic interventions including therapeutic exercise, gait training, manual therapy, neuromuscular re-education, and relevant modalities within scope.
- Reassess progress, modify treatment plans, and set functional goals to support discharge planning.
- Document evaluations, treatment plans, progress notes, and discharge summaries accurately in the designated electronic medical record system.
- Collaborate with an interdisciplinary team to align goals, coordinate patient care, and ensure safety and infection control practices.
- Educate patients and families on exercises, precautions, fall prevention, and home exercise programs to support continued recovery after discharge.
Requirements
- Education: Doctor of Physical Therapy (DPT) degree; DPT required if obtained after 2016.
- Certifications: BLS (Basic Life Support) certification required.
- Experience: Home Health experience preferred (1 year would suffice); new graduates may be considered with appropriate training.
- Age: Must be 21 years or older.
- Additional: Ability to work in a hospital-based setting and follow established PT protocols and safety guidelines.
