Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions, aligned with department objectives.
- Assess patient needs, develop and revise individualized treatment plans, and monitor progress.
- Deliver outpatient rehabilitation interventions to improve mobility, strength, balance, endurance, and functional independence within a home-based hospice context.
- Document evaluations, progress notes, and outcomes accurately; maintain compliant records.
- Serve as co-leader in an interdisciplinary approach to patient groups, coordinating care with nurses, social workers, and other clinicians.
- Communicate clearly with patients and families about goals, progress, and plan of care; adjust plans as clinically indicated.
Requirements
- Active Physical Therapy license in California
- Doctor of Physical Therapy (DPT) degree from an accredited program
- Relevant clinical experience in outpatient or home health/hospice settings
- CPR/BLS certification or equivalent (where required)
- Strong collaboration and communication skills for interdisciplinary teams
- Ability to work in a home-based outpatient setting; willingness to participate in travel assignments
