Responsibilities
- Assess residents for speech, language, cognitive, and swallowing disorders; document findings.
- Develop and implement individualized treatment plans targeting communication, cognition, and safe swallowing.
- Deliver direct therapy to residents and monitor progress, adjusting interventions as needed.
- Conduct screenings and refer for further evaluation when appropriate; maintain accurate patient records.
- Collaborate with an interdisciplinary team to optimize patient outcomes; communicate with families and caregivers about strategy and progress.
- Participate in care planning and discharge planning as part of the resident’s overall wellbeing.
