Responsibilities
- Provide direct patient care under supervision of a registered nurse, following scope of practice for LPNs.
- Administer medications and treatments per physician orders and document administration accurately.
- Monitor patient status, record vital signs, assess changes in condition, and report promptly to the supervising nurse.
- Assist with activities of daily living and patient mobility, maintaining dignity and safety.
- Collaborate with the interdisciplinary care team to update care plans and document care delivery in the medical record.
- Enforce infection prevention protocols, safety standards, and proper use of equipment; respond to call bells and assist with falls prevention.
Requirements
- Active Vermont Licensed Practical Nurse license in good standing
- Current CPR/BLS certification
- Graduation from an approved practical nursing program (LPN diploma or degree)
- Relevant long-term care or senior care experience preferred
- Ability to work day and evening shifts and perform travel assignments
- Strong communication skills and ability to document accurately in the medical record
