Responsibilities
- Administer prescribed medications and treatments per physician orders; document in MAR/eMAR and nursing notes
- Monitor residents’ health status, perform focused assessments, and report changes to the supervising RN
- Assist with activities of daily living, personal care, mobility support, and dining assistance
- Perform routine nursing tasks such as wound care, injections, vital signs, and specimen collection per facility protocol
- Maintain accurate documentation and update care plans; participate in shift handoffs and care coordination
- Maintain safety, infection control, and privacy standards; collaborate with the care team to meet individualized care plans
Requirements
- Active Licensed Practical Nurse license in New Hampshire
- Current CPR/BLS certification
- Graduate of an accredited LPN program; eligible for NH licensure
- Prior long-term care or skilled nursing facility experience is preferred
- Ability to work evenings and weekends as required by the shift; reliable and collaborative team member
- Strong communication skills and attention to detail; ability to document accurately
