Responsibilities
- Administer prescribed medications and treatments within the scope of practice for an LPN
- Monitor residents’ health, perform routine assessments, document findings, and report changes to the supervising nurse
- Assist with activities of daily living and ongoing nursing care, ensuring safety and comfort
- Provide wound care and dressing changes as directed by the care plan
- Collaborate with the interdisciplinary team and communicate with families as appropriate
- Maintain infection control practices, accurate documentation, and compliance with facility policies
Requirements
- Active Licensed Practical Nurse license in New Hampshire or eligibility to practice in NH
- LPN diploma or equivalent education
- Ability to work evenings (3:00 PM–11:30 PM) and maintain a 40-hour week
- Local candidate to Keene, NH area
- Prior long-term care or skilled nursing experience preferred
- Willingness to complete credentialing requirements and background checks as required
