Responsibilities
- Provide direct patient care to residents under supervision of a registered nurse in a long-term care setting
- Administer medications and track treatments per orders and facility protocols
- Monitor resident status, perform routine assessments, and document findings
- Assist with activities of daily living (ADLs) and promote resident independence
- Maintain accurate, timely documentation in the chart/EHR and communicate changes to the care team
- Uphold safety, infection control practices, and care planning participation; collaborate with interdisciplinary teammates
Requirements
- Active Licensed Practical Nurse license (VT) or eligibility to practice in Vermont
- Current BLS/CPR certification
- Prior experience in long-term care or geriatric settings preferred
- Ability to work 40 hours per week; available for day and/or evening shifts
- Strong communication, teamwork, and critical-thinking skills
