Responsibilities
- Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wounds; develop and implement a plan of care in line with regulatory and clinical standards.
- Administer rehabilitation procedures, including manual techniques; ambulation training; therapeutic exercises; modalities; use of assistive devices; wound care as indicated.
- Assess patient progress, adjust treatments as needed, and document outcomes in the electronic clinical record per regulatory requirements.
- Provide patient, family, and caregiver counseling and education; communicate progress, barriers, and treatment plans with the supervising clinician and health team.
- Prepare comprehensive discharge summaries and participate in patient care conferences, utilization reviews, and rehabilitation meetings; maintain timely documentation.
- Supervise and support physical therapist assistants, rehabilitation aides, and/or students; adhere to APTA and state practice acts and ethical standards.
Requirements
- Missouri Physical Therapy license (active) required.
- Minimum 1 year of physical therapy experience; LTC or rehab experience required.
- New graduates OK if they have Level II clinical experience (not limited to observation).
- Ability to work in a home health setting (travel assignment); strong documentation and communication skills.
- Must comply with professional standards, including APTA/state practice acts and ethics.
