Responsibilities
- Conduct comprehensive physical therapy evaluations and develop individualized treatment plans for residents in the SNF setting.
- Deliver evidence-based PT interventions to address gait, balance, strength, range of motion, and functional mobility to improve independence.
- Monitor patient progress, adjust treatment plans as needed, and document outcomes in the facility’s charting system.
- Collaborate with an interprofessional team (nursing, physicians, therapy assistants, and families) to optimize patient outcomes.
- Instruct and educate residents and families on home exercise programs, safety strategies, and activity modification.
Requirements
- Ability to work in a skilled nursing facility environment and support an urgent-need assignment.
- Willingness to participate in a 13-week travel assignment.
- Strong communication and documentation skills; comfort working within an interdisciplinary team.
- Demonstrated professionalism and adherence to safety, infection control, and privacy standards.
- Availability to meet guaranteed weekly hours and standard shift expectations.
