- Provide direct patient care for adults in a long-term care and rehab context.
- Administer prescribed medications and document administration per policy.
- Monitor vital signs, assess changes in patient condition, and respond appropriately.
- Assist with care planning, documentation, and routine nursing tasks under supervision of the RN and multidisciplinary team.
- Collaborate with physicians, therapists, counselors, and other professionals to support safety and recovery goals.
- Maintain infection control practices and accurate charting in electronic health records.
Requirements
- Active Vermont Licensed Practical Nurse (LPN) license.
- Ability to work the overnight shift.
- Willingness to travel for a temporary assignment.
- Experience in long-term care or rehabilitation settings preferred.
- Ability to work in both outpatient and inpatient rehab environments.
- Must meet credentialing/safety requirements for travel assignments.
