Responsibilities
- Provide direct patient care to residents in a long-term care setting, including activities of daily living support, vital signs, and routine assessments.
- Administer prescribed medications and treatments in accordance with physician orders and facility policies.
- Monitor residents for changes in condition, document findings, and escalate concerns to the supervising nurse promptly.
- Collaborate with interdisciplinary team members to develop and execute individualized care plans.
- Maintain accurate documentation in the electronic health record and ensure timely communication with family and care staff.
- Prioritize safety, infection control, and patient rights; respond to emergencies and ensure a calm, compassionate approach to care.
