Responsibilities
- Conduct clinical assessments to identify patient needs and determine appropriateness of physical therapy.
- Develop, implement, and periodically revise a Plan of Care (POC) with progress notes and related documentation; collaborate with physicians and an interdisciplinary team.
- Plan and deliver therapy for neuromuscular, neurological, and orthopedic conditions based on medical referral and evaluative data.
- Instruct patients and families in safe transfer techniques and the use of equipment (walkers, wheelchairs, canes, braces, prosthetic devices); select and adapt equipment as needed for home environments.
- Provide patient and family education tailored to age, condition, culture, and learning readiness; promote infection control and universal precautions.
- Coordinate with and, when applicable, supervise other team members to ensure care aligns with patient needs and preferences.
Requirements
- Active Physical Therapist license in the state of Washington (or eligibility as required) – Required
- Current CPR certification – Required
- Valid driver’s license with reliable transportation and adequate auto insurance – Required
- 1–2 years of PT experience in acute care or rehabilitation settings – Required
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association–approved program – Required
- Ability to work in a home healthcare setting and travel locally as needed
