Responsibilities
- Provide skilled physical therapy services to patients in their homes, including comprehensive evaluation and treatment planning.
- Develop and implement individualized home health plans of care; monitor patient progress and adjust as needed.
- Educate patients and families on exercises, safety, self-management, and the use of assistive devices.
- Document evaluations, treatments, goals, progress, and communications with the care team in the patient record.
- Collaborate with an interdisciplinary team to coordinate care and ensure compliance with state and federal home health regulations.
Requirements
- Active Pennsylvania physical therapy license
- Doctor of Physical Therapy (DPT) degree or equivalent from an accredited program
- Ability to practice in a home health setting and travel within the service area to patient homes
- Home health experience preferred but not required
- Strong communication, documentation, and organizational skills
