Responsibilities
- Provide direct nursing care to long-term care residents within the LPN scope of practice
- Monitor, document, and report vital signs, symptoms, and changes in resident condition
- Administer medications and treatments per physician orders, with accurate charting and DEA/compliance as applicable
- Assist residents with activities of daily living and mobility, promoting safety and independence
- Identify and respond to resident needs, prioritize tasks, and collaborate with the interdisciplinary team
- Maintain infection prevention, safety protocols, and patient privacy in accordance with regulations and facility policy
Requirements
- Active LPN license in good standing (state of practice: New Hampshire) or eligibility to obtain
- Current CPR/BLS certification
- Ability to perform 12-hour shifts and a rotating schedule as described
- Experience in long-term care or skilled nursing facility settings preferred
- Strong clinical assessment, documentation, and communication skills
