Responsibilities
- Provide skilled physical therapy evaluation and treatment planning for residents in a skilled nursing setting.
- Administer therapeutic exercises, gait training, balance activities, ROM, and pain management as prescribed.
- Monitor patient progress, adjust treatment plans, and document outcomes in charts in collaboration with the interdisciplinary team.
- Educate residents and families on safety, home exercise programs, and discharge planning as appropriate.
- Ensure patient safety, proper use of modalities and equipment, and adherence to infection control and documentation standards.
- Communicate regularly with nursing staff, occupational therapy, speech-language pathology, and social work to optimize functional outcomes and transitions of care.
Requirements
- Eligibility to practice physical therapy in the United States or eligible for licensure.
- Availability to begin on 06/22/2026 and commit to a 13-week assignment.
- Ability to work a fixed 8-hour day shift (08:00–16:30) with guaranteed hours.
- Experience or comfort working in a skilled nursing facility or long-term care setting.
- Strong documentation, communication, and teamwork skills; adaptability to travel assignments.
