Licensed Practical Nurse - Long-Term Care (Travel) – Geneva, NY, United States. Start date: 06/09/2026. Assignment duration: 14 weeks. Weekly pay: $1,386-$1,407. Shift: evenings, 5x8. The LPN will deliver direct and indirect skilled nursing care to adult and geriatric residents under the supervision of a Registered Nurse, participate in the interdisciplinary team, and contribute to age-appropriate care plans.

Responsibilities
- Provide direct and indirect skilled nursing care to assigned residents within a long-term care population.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and Interdisciplinary Team.
- Contribute to development of an age-appropriate, culturally relevant plan of care; involve resident/family and healthcare team in care planning.
- Recommend revisions to the plan of care based on resident outcomes and coordinate referrals to ensure comprehensive care.
- Collaborate with residents, families, and team members; follow policies and procedures under RN supervision; participate in ongoing education and in-service activities.
- Maintain current knowledge and competency in nursing practice; assist with mentoring or orientation of staff as needed; uphold safety standards and infection control protocols.
Requirements
- Active New York LPN licensure
- Basic Life Support (BLS) certification (AHA or RQI)
- Minimum 1 year of LPN experience in a long-term care setting
- Ability to provide care to adult and geriatric residents
- Capability to work in a skilled nursing facility environment
- Eligible to participate in a travel assignment and meet facility onboarding requirements
