Responsibilities
- Provide direct patient care to residents under the supervision of an RN in a long-term care setting.
- Administer prescribed medications and document administration accurately.
- Monitor and assess patients, report changes in condition, and respond to changes per protocol.
- Perform routine assessments, wound care, skin integrity checks, and assist with activities of daily living.
- Document care plans, vital signs, intake/output, and any changes in condition; communicate clearly with the care team.
- Ensure patient safety, infection control, and adherence to facility policies and procedures.
Requirements
- Current, active LPN license (unrestricted) suitable for practice in the United States.
- Ability to work night shifts and the 12-hour shift length (3x12) required by the assignment.
- Experience in long-term care or skilled nursing settings preferred.
- Strong communication and documentation skills; ability to collaborate with a multidisciplinary team.
- Commitment to patient safety and quality of care; able to follow facility policies and procedures.
