Responsibilities
- Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.
- Obtain data on physical, psychological, social, spiritual, and cultural factors and incorporate that data into the Plan of Care.
- Maintain up-to-date patient records so that visits, contacts, problems, plans, actions, and goals are accurately stated and updated as changes occur.
- Provide care utilizing infection control measures and educate patients and families regarding care, medications, home safety, and self-care practices.
- Supervise and provide clinical directions to HHA/LVN to ensure quality and continuity of service.

Requirements
- Active Missouri Registered Nurse license (MO RN) – required.
- CPR certification – required.
- 1-2 years RN clinical care experience; 2+ years home health experience – required.
- Education: Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program; Bachelor of Science in Nursing – BSN preferred.
- Certifications: CHPN certification preferred.
- OASIS experience and Homecare Homebase software experience required.
- Knowledge of venipuncture and IV care; competence in wound/decubiti care, inhalation therapy, phlebotomy for outpatient testing, and bowel/bladder training programs.
- Current driver's license and reliable transportation; ability to travel locally and work a flexible schedule.
- Strong communication and documentation skills; ability to work independently and as part of an interdisciplinary team.
