Responsibilities
- Conduct initial PT evaluations, reassessments, and establish an individualized plan of care for residents
- Develop and implement treatment plans focused on mobility, gait training, strength, balance, and activities of daily living
- Administer therapeutic exercises, modalities, and functional training to improve or restore function
- Monitor progress, adjust plans as needed, and document findings clearly in the electronic health record
- Collaborate with an interdisciplinary team (nursing, therapy assistants, social work) to support comprehensive resident care and discharge planning
- Educate residents and families on home exercise programs, safety, and ongoing self-management; ensure continuity of care on transitions of care
- Maintain patient safety during therapeutic activities and ensure compliance with relevant regulations and facility policies
Requirements
- Active Physical Therapist license in Maryland or current eligibility for licensure in Maryland
- Doctor of Physical Therapy (DPT) degree from an accredited program
- 1+ year of physical therapy experience in skilled nursing facility or geriatric rehabilitation preferred
- Ability to deliver 35+ hours per week; availability for a 5-day, day-shift schedule
- Proficient documentation and comfort with typical PT EHR workflows
- CPR/BLS certification
