Responsibilities
- Conduct in-person participant assessments to determine the appropriate level of care per the plan of care; classify as emergent, urgent, or non-urgent and respond to calls from PCP and SCPP staff.
- Deliver regular field visits to provide nursing care, including wound care, IV hydration, lab draws, and routine and acute nursing tasks as ordered by the PCP.
- Document all interactions in the EMR; initiate emergency treatment when needed and update orders and PUIs as indicated.
- Educate participants, DPOA, family, and facility staff; coordinate with providers, SCPP staff, DME suppliers, ALF/SNF, paramedics, pharmacy, administration, and home care coordinators to ensure comprehensive support.
- Complete documentation for incidents (falls, medication errors, suicide risk, etc.) and participate in quality improvement activities within a participant-centered care framework.
- Actively participate in orientation, ongoing in-service programs, staff meetings, continuous quality improvement, and performance/protocol evaluations; adapt to changing workloads and keep leadership informed.

Requirements
- Current Michigan RN license (RN Nursing Diploma or Degree from an accredited school; BSN preferred, ADN required per options).
- Education: Bachelor’s degree preferred; Associate’s degree required.
- Minimum 1 year of experience with frail, elderly, or long-term care populations; 1 year home health experience preferred; 1 year wound care experience preferred.
- CPR certification by American Heart Association or willingness to obtain.
- Must possess a current valid Michigan driver’s license, reliable transportation, and car insurance.
- Must be 18 years of age or older and fully vaccinated, or willing to be fully vaccinated against influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B.
- Ability to perform in a home health/CCRC setting and meet the travel requirements of a travel assignment.
