Responsibilities
- Plan, organize, and direct home care services to meet patient needs in their homes
- Manage a patient caseload in an effective and efficient manner
- Provide patient care independently, while collaborating with core disciplines to achieve care goals
- Serve as a resource to patients, family members, and other care team members
- Document assessments, care plans, and progress; communicate with the care team to adjust plans as needed
- Support continuity of care through coordination across services and settings
Requirements
- Active RN license
- Experience in care management or home-based patient care is typical for this role (as described in the responsibilities)
- Ability to work independently while collaborating with an interdisciplinary team
- Strong communication and documentation skills
- Comfortable delivering care in a home health setting and coordinating with patients and families
- Availability to operate within a day shift schedule
