Responsibilities
- Provide direct and indirect skilled nursing care to assigned residents under RN supervision.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and interdisciplinary team.
- Contribute to the development and execution of an age-appropriate, culturally relevant plan of care, involving the resident and family and coordinating with the healthcare team.
- Monitor resident outcomes and recommend revisions to the plan of care; initiate referrals as needed to ensure comprehensive care.
- Maintain current knowledge of nursing practice; participate in continuing education, inservice programs, and staff mentoring; demonstrate professional leadership in the practice setting.
- Administer prescribed medications and treatments under the direction of the RN and facility policy; uphold safety, infection control, and quality standards.

Requirements
- Active New York LPN license
- BLS certification (American Heart Association or RQI)
- Minimum 1 year of LPN experience, preferably in a long-term care setting
- Ability to work nights on a 2x8 schedule; every other weekend Fri & Sat night and holidays as required
- Comfort working with adult and geriatric residents in a skilled nursing environment
- Effective communication and collaboration with the interdisciplinary team; ability to work under RN supervision
