Responsibilities
- Conduct comprehensive PT evaluations and periodic re-evaluations for residents to determine functional status and goals.
- Develop, implement, and adjust individualized treatment plans focused on mobility, strength, balance, and activities of daily living.
- Deliver evidence-based interventions including therapeutic exercise, gait training, manual therapy, and functional training.
- Monitor patient progress, document outcomes, update care plans, and communicate with the interdisciplinary team to optimize resident care.
- Ensure patient safety, infection control, and adherence to facility policies during therapy sessions and documentation.
- Support discharge planning and provide patient- and caregiver-appropriate home exercise programs and education.
Requirements
- Active Physical Therapist license in Pennsylvania (in good standing)
- Doctor of Physical Therapy (DPT) degree from an accredited program
- Willingness to undertake a 13-week travel assignment and work a 5x8 schedule
- Ability to work effectively in a geriatric, long-term care setting with a diverse resident population
- Strong clinical documentation skills and ability to collaborate with an interdisciplinary team
- Eligibility for credentialing requirements and agency onboarding processes
