Responsibilities
- Conduct comprehensive occupational therapy evaluations for residents and develop individualized treatment plans to improve activities of daily living, mobility, and functional independence.
- Implement evidence-based interventions, monitor progress, and adjust plans to maximize functional outcomes.
- Collaborate with the interprofessional team including nursing, speech therapy, physical therapy, social work, and case management to ensure coordinated care.
- Document evaluation findings, treatment progress, and discharge recommendations in the resident’s medical record; communicate with families and care teams as appropriate.
- Participate in discharge planning and provide caregiver education to support ongoing rehabilitation in the home or care setting.
Requirements
- Active occupational therapy license in Vermont or eligibility to obtain licensure per state requirements
- Ability to work effectively in a skilled care setting and with an interprofessional team
- Strong documentation and communication skills
- Willingness to participate in a 13-week travel assignment and adhere to the assigned shift schedule
