Responsibilities
- Provide direct patient care to residents within the scope of an LPN, under the direction of the supervising nurse
- Administer medications and document accurately in the resident chart and electronic health record
- Monitor vital signs, observe for changes in condition, and report findings to the RN or physician
- Assist with activities of daily living and support resident safety, mobility, and comfort
- Complete routine procedures within scope, including basic wound care and dressing changes as directed
- Collaborate with an interdisciplinary team to coordinate care plans and ensure smooth shift handoffs
Requirements
- Active Licensed Practical Nurse (LPN) license in good standing
- Completion of an approved LPN program
- Prior experience in long-term care or skilled nursing settings preferred
- Ability to work night shifts and weekends as assigned
- Strong assessment, documentation, and communication skills
- Capable of functioning effectively in a fast-paced, team-based environment
