Responsibilities
- Administer medications and treatments as prescribed and document administration per protocol
- Monitor residents’ health status, observe for changes, and report findings to the supervising RN
- Provide direct patient care including assistance with activities of daily living (ADLs)
- Perform routine assessments, vital signs, and documentation in the charting system
- Maintain infection control practices, safety, and accurate care planning
- Collaborate with the interdisciplinary team to ensure quality resident care and timely communication
Requirements
- Active LPN license in Vermont
- Graduate of an approved practical nursing program (or equivalent)
- Ability to work in a long-term care setting and perform the listed duties
- Availability for mostly evening shifts with potential float to other shifts
- Strong communication, documentation, and teamwork skills
- Must meet regulatory and facility requirements for travel assignments
