Responsibilities
- Perform initial evaluations and ongoing reassessments to establish and adjust individualized physical therapy plans
- Implement evidence-based PT interventions to restore and improve physical function and mobility
- Collaborate with the interdisciplinary care team to support patient goals and discharge planning
- Document evaluations, treatment plans, progress notes, and discharge summaries in a timely, accurate manner
- Supervise and delegate tasks to assigned therapy support personnel as appropriate
- Maintain patient safety and regulatory compliance during all treatment activities

Requirements
- Doctor of Physical Therapy (DPT) degree; required if license was obtained on or after January 2016; foreign equivalents considered
- Physical Therapy licensure in the applicable state(s) of practice; active CA license or eligibility preferred for this assignment
- Minimum two years of recent experience in physical therapy
- CPR certification or Basic Life Support (BLS) certification from an approved vendor
- Valid driver’s license and must be at least 21 years of age
- EPIC electronic medical record experience
- Ability to work in a hospital-based setting
