Responsibilities
- Provide direct, resident-centered nursing care according to the individualized care plan
- Administer prescribed medications and document responses and health status
- Monitor vital signs, assess changes, and report findings to the supervising nurse or physician
- Assist residents with activities of daily living and support mobility and safety needs
- Maintain accurate, timely documentation in the electronic health record and care notes
- Collaborate with the interdisciplinary team to ensure continuity of care and quality outcomes
Requirements
- Active LPN license in Pennsylvania (in good standing)
- Nursing experience in long-term care or a similar setting is preferred
- Ability to work Day or Evening shifts with guaranteed 40-hour weeks
- Willingness to participate in a travel temporary assignment
- Strong communication and teamwork skills, with a resident-centered care approach
