Occupational Therapist, Acute Care – Clearlake, CA. Travel assignment starting 06/08/2026 for 13 weeks in a critical access, acute care setting. Shift is 5x8 days (day shift). Responsibilities
- Perform OT evaluations and deliver interventions for adult patients in an acute care context, focusing on activities of daily living, mobility, and cognitive function.
- Develop, implement, and adjust individualized plans of care with measurable goals; document progress and outcomes.
- Utilize HCHB documentation for care planning, progress notes, and discharge planning; coordinate with the care team to ensure seamless transitions.
- Collaborate with physicians, nurses, and therapy colleagues to optimize patient outcomes and safety.
- Provide patient and caregiver education on therapeutic activities, home exercise programs, and safety post-discharge; participate in discharge planning as appropriate.
- Maintain compliance with clinical policies and AHA BLS requirements; support quality and safety initiatives.
- Operate in a daytime schedule with no on-call or weekend responsibilities.
Requirements
- Minimum 2 years of occupational therapy experience; at least 3 months of recent experience in the specialty.
- Proficiency with HCHB and at least 1 year of home health OT experience.
- AHA BLS certification required.
- Must be 21 years of age or older.
- Submission should include preferred start date, RTO, SSN, and DOB.
