Responsibilities
- Provide occupational therapy evaluations and interventions for adult patients, following the established plan of care and client goals.
- Develop, implement, and adjust therapy plans focusing on activities of daily living, therapeutic activities, mobility, and compensatory strategies; track progress and update goals as needed.
- Document thoroughly using HCHB and maintain accurate, timely records to meet regulatory and payer requirements.
- Collaborate with an interdisciplinary team, including nursing, social work, and family/caregivers, to coordinate comprehensive patient care.
- Educate patients and families on safe techniques, equipment use, energy conservation, and home modifications to support independence.
- Reassess patient progress, prepare outcomes and discharge summaries, and ensure continuity of care across settings.
Requirements
- Active occupational therapy license (California) or eligibility to practice per state requirements
- Minimum 2 years of occupational therapy experience; at least 3 months of recent experience in the specialty
- At least 1 year of home health experience as an OT
- Knowledge and experience with HCHB; ability to document in Home Health settings
- BLS/CPR certification (AHA) required
- Age 21 or older
- Ability to work in a day-shift setting (Mon-Fri, 08:00–16:00)
Education
- Graduation from an accredited occupational therapy program (degree level per state requirements)
