Responsibilities
- Provide direct nursing care to adult residents in a skilled nursing/long-term care setting under supervision of a registered nurse
- Administer prescribed medications and treatments in accordance with physician orders and facility policy
- Monitor residents’ health status, record vital signs, and identify and report changes promptly to the supervising RN
- Assist residents with activities of daily living, mobility, hygiene, feeding, and comfort measures
- Document care interventions, vitals, medications, and resident responses accurately in the electronic health record
- Collaborate with the interdisciplinary team to implement and adjust care plans, ensuring resident safety and quality of life
Requirements
- Active LPN license (state requirements aligned with patient setting) and ability to practice in the applicable jurisdiction
- Minimum of relevant professional experience in a long-term care or skilled nursing setting preferred
- Ability to work night shifts and meet guaranteed hours
- Strong clinical skills in medication administration, documentation, and resident safety
- Effective communication and teamwork skills; ability to respond to changes in resident condition promptly
- Willingness to participate in a travel assignment and comply with facility policies and procedures
