Responsibilities
- Provide direct and indirect resident care under the direction of licensed nursing personnel, aligned with the plan of care.
- Assist residents with activities of daily living, including hygiene, grooming, dressing, incontinence management, mobility, turning and positioning, and skin care; document per protocol.
- Maintain a safe and clean environment; respond to call lights and alarms promptly; monitor safety devices and report issues per policy.
- Support nutritional needs, including meal delivery, tray setup, feeding assistance, and documenting intake as required.
- Support rehabilitative/restorative needs through range of motion activities, transfers, ambulation, proper body alignment, and use of assistive devices; document progress.
- Respect residents’ rights, preferences, refusals, and psychosocial needs; escalate changes in status to the Charge Nurse promptly.
Requirements
- Active New York State Nursing Assistant Certification (NYS CNA)
- BLS certification (AHA or RQI)
- Minimum 1 year of experience in long-term care or CNA capacity
- Ability to perform ADL assistance, transfers, and routine nursing tasks
- Ability to work in a skilled nursing facility setting and to participate in a rotating schedule
- Willingness to work weekends and holidays as required
