Responsibilities
- Assess patient needs and develop comprehensive case management plans across multidisciplinary teams.
- Coordinate care across inpatient, outpatient, and community services; facilitate timely discharges and appropriate referrals.
- Monitor patient progress, identify barriers to care, and adjust plans to optimize outcomes and resource use.
- Document care plans, updates, and discharge instructions in the electronic medical record; communicate with physicians, patients, and families.
- Advocate for patients and families, provide education about treatment plans, and support transitions of care.
Requirements
- Current New York Registered Nurse license
- Basic Life Support (BLS) certification
- 1+ year of RN experience; case management or care coordination experience preferred
- Bachelor’s degree in Nursing (BSN) or ADN with relevant case management experience
- Strong communication, collaboration, and documentation skills; ability to work in a hospital travel setting; proficient with EMR
