Responsibilities
- Assess patients in the home setting to determine physical therapy needs and goals

- Deliver PT interventions, including therapeutic exercises, manual therapy, and gait training
- Monitor progress, adjust plans as needed, and document results in patient records
- Educate patients and families on home exercise programs, safety, and fall prevention
- Communicate and collaborate with the interdisciplinary care team to ensure coordinated care
Requirements
- Active Physical Therapist license in Pennsylvania or eligibility for licensure
- Doctor of Physical Therapy (DPT) degree or equivalent
- Ability to provide patient-centered care in a home health setting
- Strong communication and documentation skills
- Ability to travel to patient homes within the service area as required
