Responsibilities
- Deliver coordinated nursing care for an individual patient or an assigned group of patients according to established standards of care and the nursing process.
- Supervise and direct activities of various levels of assigned nursing staff; coordinate care with other disciplines while applying critical thinking and independent judgment.
- Perform patient assessments, develop and implement care plans, and evaluate patient outcomes in a home healthcare – hospice setting.
- Document care and maintain accurate patient records; ensure continuity of care across shifts and settings.
- Collaborate with interdisciplinary teams to support patient and family education, comfort, and quality of life at end of life.
- Ensure patient safety, regulatory compliance, and appropriate utilization of resources.
Requirements
- Active RN license in the United States with eligibility for California licensure or licensure by endorsement.
- ER nursing experience preferred.
- Ability to work nights and 12-hour shifts in a home healthcare – hospice environment.
- Strong communication and teamwork skills; ability to coordinate care with diverse care teams.
- Competence in triage, assessment, and care planning; documentation accuracy.
- Willingness to comply with agency policies and state-specific travel nursing requirements.
