Responsibilities
- Manage an assigned caseload of hospice patients and lead the interdisciplinary team to coordinate ongoing care.
- Conduct comprehensive assessments, identify personal goals of care with patients and families, and educate caregivers on patient needs.
- Provide symptom management, comfort-focused interventions, and timely response to changes in patient status in collaboration with the medical team.
- Develop and document care plans, progress notes, and communications; coordinate with physicians, social workers, and home health staff.
- Deliver nursing care in patients’ homes and other community settings; adapt plans for disease progression and changing family circumstances.
- Participate in weekend coverage and on-call as needed to ensure continuity of care and support for patients and families.
Requirements
- Active, current Colorado RN license; CPR/BLS certification required.
- Experience in hospice or end-of-life, home-based care preferred.
- Ability to travel to patient homes and community settings; reliable transportation and a valid driver’s license.
- Strong assessment, critical thinking, and interpersonal skills; ability to lead an interdisciplinary team and communicate effectively with patients, families, and colleagues.
- Education: Graduate of an accredited nursing program; BSN preferred.
