Responsibilities
- Provide direct and indirect skilled nursing care for assigned adult and geriatric residents under the supervision of a Registered Nurse.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and Interdisciplinary Team.
- Contribute to the development and revision of an age-appropriate, culturally relevant plan of care, involving the resident, family, and healthcare team to achieve outcomes.
- Reassess resident status and recommend plan modifications; refer to appropriate team members to address all aspects of care.
- Collaborate with residents/families and Interdisciplinary Team to ensure a safe, quality care environment; mentor and orient other staff as needed.
- Maintain current nursing knowledge and competencies; demonstrate leadership in the practice setting under RN direction.
Requirements
- Active NY Licensed Practical Nurse license
- BLS certification (through AHA or RQI specifically)
- Minimum 1 year of LPN experience, preferably in a long-term care setting
- Ability to provide care for adult and geriatric residents and work in a long-term care environment
- Weekend and holiday availability is required
- Flexibility to work evenings and the outlined 3x8 shift schedule
