Responsibilities
- Provide direct nursing care to adult residents in a long-term care setting, including those with multiple chronic conditions (e.g., MS, ortho, dementia, memory care)
- Administer medications and monitor patient responses in accordance with physician orders and facility policies
- Perform activities of daily living assistance and routine clinical tasks under the supervision of a registered nurse
- Monitor vital signs, assess changes in patient status, document appropriately, and escalate concerns per care plans
- Document care and interventions in PointClickCare; maintain accurate and compliant records
- Float to all units within the same building as needed and participate in interdisciplinary teamwork to ensure resident safety and quality of care
Requirements
- Active New York LPN/LVN license in good standing
- BLS certification
- Minimum 1 year of LPN/LVN experience in long-term care or skilled nursing
- Ability to float to all units within the same building
- Proficiency with PointClickCare or willingness to learn
- Availability for the 6:30 a.m.–2:30 p.m. shift, with every-other-weekend requirements
