Responsibilities
- Provide direct patient care under supervision of registered nurses and physicians.
- Administer prescribed medications and treatments; document administration and monitor for adverse effects.
- Conduct routine assessments, monitor vital signs, and update patient charts.
- Assist with activities of daily living and provide restorative care to support functional goals.
- Maintain accurate documentation and adhere to infection control and safety protocols.
- Communicate effectively with the interdisciplinary care team and respond promptly to patient needs.
Requirements
- Active LPN license in West Virginia.
- Graduation from an accredited practical nursing program.
- Experience in long-term care or geriatric settings preferred.
- Ability to work the 3x12 night shift and participate in a travel assignment.
- Ability to pass onboarding, credentialing, and background screening requirements.
