Responsibilities
- Administer prescribed medications and document administration accurately per clinical policies; monitor residents for adverse reactions and interactions, reporting concerns to the supervising RN.
- Deliver direct patient care, including activities of daily living, vital signs, routine assessments, wound care, and assistance with mobility and transfers to promote comfort and safety.
- Observe and report changes in resident condition; participate in care planning discussions and interdisciplinary rounds to coordinate individualized care plans.
- Maintain precise and timely charting in electronic health records; uphold privacy, confidentiality, and infection control standards in all routine tasks.
- Ensure a safe environment by adhering to falls prevention, mobility safety protocols, proper device management, and prompt response to call lights and emergencies.
- Collaborate with licensed staff, CNAs, and other team members to implement care plans, educate family members when appropriate, and advocate for residents’ dignity and preferences.
Requirements
- Active Licensed Practical Nurse license in West Virginia
- Current CPR/BLS certification
- Completion of an approved practical nursing program (LPN)
- Ability to work a 3x12-hour night schedule and participate in a temporary travel assignment
- Prior long-term care experience preferred, with demonstrated ability to support geriatric residents in a nursing home setting
