Responsibilities
- Deliver bedside nursing care to residents per plan of care, including vital signs, medication administration, and documentation.
- Monitor residents for changes in condition and report to supervising RN; adjust care plans as directed.
- Administer medications and treatments per physician orders and facility policies.
- Collaborate with the interdisciplinary team to implement and update individualized care plans.
- Assist residents with activities of daily living and promote safety and dignity.
- Document care delivery accurately in the electronic health record and communicate promptly with the team.
Requirements
- Active Licensed Practical Nurse license in New Hampshire.
- Graduation from an accredited practical nursing program.
- Experience in a long-term care or skilled nursing setting is preferred.
- Availability to work the assigned schedule, including evenings and weekends, for the 5-week contract.
- Ability to meet the physical demands of a long-term care environment and work effectively within a clinical team.
