Responsibilities
- Administer medications and monitor residents’ conditions; document care accurately
- Perform routine assessments and report changes to the supervising nurse or physician
- Assist residents with activities of daily living (bathing, dressing, mobility)
- Provide wound care, comfort measures, and routine clinical procedures as delegated
- Maintain safety and infection control protocols; supervise fall prevention and patient transfers
- Collaborate with the interdisciplinary team to update and implement individualized care plans
Requirements
- Active Licensed Practical Nurse license in New Hampshire
- Graduation from an approved practical nursing program
- Prior long-term care or skilled nursing experience preferred
- Ability to work evenings (3:00 PM–11:30 PM) and every other weekend
- Strong clinical assessment, documentation, and communication skills
- Ability to thrive in a fast-paced LTC environment and work collaboratively with the team
